Who is this vehicle inspection checklist for?

  • Office and fleet managers who look after a few to a few dozen company vehicles
  • Safety coordinators who want drivers to check a vehicle before they take it out
  • Care homes, schools, and contractors that run shuttle vans or work trucks

When to use it

The driver fills in the Daily Pre-Trip Check before the first trip of the day, one sheet per vehicle per month. Keep it in the vehicle or at the key box. Use the Trip and Mileage Log when several drivers share vehicles at one location and you want each trip and its mileage on one line.

The template includes the following. Delete anything you do not need.

  • Daily Pre-Trip Check: 21 items in 3 groups (under the hood, around the vehicle, driver's seat)
  • Driver initials and odometer reading for each day

  • Trip and Mileage Log: 6 quick checks by area, time out and in, odometer out and in, and miles driven calculated automatically

42Related regulations and standards (for reference): 49 CFR 396.11(a) applies to commercial motor vehicles under FMCSA rules (with some exceptions): drivers prepare a written driver vehicle inspection report at the completion of each day's work on each vehicle operated. 49 CFR 396.13(a): before driving, the driver must be satisfied that the motor vehicle is in safe operating condition. This checklist is meant for company cars, pickups, and vans; it is not a DVIR form.
43Which inspections you need, how often, and how long to keep records depend on your equipment, industry, and location. Check the regulations that apply to you, the manufacturer's instructions, and a qualified person.

How to use the vehicle inspection checklist

  1. Make one copy of the file for each vehicle and fill in the vehicle ID, plate, and model.
  2. Before the first trip of the day, the driver checks each item and marks ✓, X, or N/A in that day's column.
  3. If any item is X, write it in the defect record and tell the person who manages the vehicle before driving.
  4. For shared vehicles, add one line per trip to the Trip and Mileage Log. Miles driven fill in by themselves.
  5. At the end of the month, print or save as PDF and start a new sheet.

Adapting it to your site

  • Delete lines that do not fit your vehicles, for example the battery fluid line for sealed batteries.
  • Add lines for equipment you carry, such as a ladder rack, a liftgate, a backup camera, or a wheelchair lift.
  • Some items, such as fluid levels, can be checked once a week instead of daily. Mark N/A on the other days.

Before you use it

Please read
  • This is a general template for company vehicles, not an official form. It is not a driver vehicle inspection report (DVIR) form for commercial motor vehicles.
  • For commercial motor vehicles, 49 CFR 396.11(a) asks drivers to prepare a written inspection report at the end of each day's work, and 49 CFR 396.13(a) asks the driver to be satisfied the vehicle is safe before driving. Check whether your vehicles fall under FMCSA rules.
  • A daily check does not replace scheduled maintenance. Follow the maintenance schedule in the owner's manual.
  • State rules and your insurer may have their own requirements. Check what applies to your vehicles.
  • This template is provided as-is for reference. Worker Library is not liable for any loss arising from its use. Regulations change, so check the update date on this page.

Vehicle Inspection Checklist FAQ

Is this the same as a DVIR?

No. A DVIR is the driver vehicle inspection report that 49 CFR 396.11(a) asks drivers of commercial motor vehicles to prepare at the end of each day's work. This checklist is for company cars and vans. If your vehicles are commercial motor vehicles under FMCSA rules, use a report that matches those rules.

How often should a company vehicle be inspected?

Common practice is a short check by the driver before the first trip of each day the vehicle is used, plus maintenance on the schedule in the owner's manual. Your company policy or insurer may set a different interval.

What should a daily vehicle inspection include?

Most daily checks cover fluid levels, tires, lights, brakes, wipers, horn, mirrors, and seat belts. This template has 21 items in three groups, and you can delete or add lines to match your vehicles.

Can I print the checklist and keep it in the vehicle?

Yes. Each sheet has a print layout for US Letter. One printed page covers a full month for one vehicle.
