Who is this forklift inspection checklist for?

  • Warehouse and plant managers who run electric or IC forklifts
  • Safety coordinators who need a daily record operators will actually fill in
  • Maintenance teams that want the monthly check and the service history in one place

When to use it

Operators fill in the Daily Pre-Shift sheet before the truck is placed in service each day, or after each shift if it runs around the clock. Maintenance uses the Monthly Inspection sheet once a month, and the Annual Service Record each time a technician does planned maintenance. Use one file per truck.

How to use the forklift inspection checklist

  1. Make one copy of the file for each truck and fill in the truck ID and model.
  2. Before the first use of the day, the operator checks each item and marks ✓, X, or N/A in that day's column.
  3. If any item is X, the truck is not used. Write the defect in the defect record and tell the supervisor.
  4. Once a month, maintenance fills in the Monthly Inspection sheet and signs it.
  5. Add a line to the Annual Service Record after each planned maintenance visit, and save the file with the year in the name.

Adapting it to your site

  • Add items your trucks have, such as a blue spot light, a load weight indicator, or a specific attachment.
  • For LPG trucks, keep the cylinder line; for electric trucks, keep the battery line. Delete the one you do not use.
  • If you run three shifts, use the day columns for each shift or make one sheet per shift.

Before you use it

Please read
  • OSHA requires powered industrial trucks to be examined before being placed in service, at least daily (29 CFR 1910.178(q)(7)). This template is a way to record that examination; it is not an official OSHA form.
  • A truck with a defect that affects safety must not be used until it is repaired (29 CFR 1910.178(q)(1)). Repairs should be done by authorized personnel.
  • Monthly and annual items here are common maintenance practice, not OSHA requirements. Follow your manufacturer's maintenance schedule.
  • State plans and your insurer may have additional requirements. Check the current rules that apply to your site.
  • This template is provided as-is for reference. Worker Library is not liable for any loss arising from its use. Regulations change, so check the update date on this page.

Forklift Inspection Checklist FAQ

How often does a forklift need to be inspected?

Under OSHA 29 CFR 1910.178(q)(7), a forklift must be examined before it is placed in service, at least once a day. If the truck is used around the clock, it must be examined after each shift. Monthly and annual inspections are set by your maintenance program and the manufacturer.

Who should do the daily forklift inspection?

In most workplaces the trained operator does the daily pre-shift check before using the truck. Repairs and planned maintenance are done by authorized maintenance personnel or a service company.

How long should I keep forklift inspection records?

OSHA 1910.178 does not set a retention period for daily checklists. Many companies keep them for at least a year, and keep maintenance and repair records for the life of the truck. Check your company policy and any state or insurer requirements.

Can I use this checklist for an electric forklift?

Yes. The checklist covers both IC (gas, diesel, LPG) and electric trucks. Lines that apply to only one type are labeled, so you can delete the ones that do not apply.
