How to use the scaffold inspection checklist

  1. Fill in the site, date, inspection type, and the competent person's name.
  2. Walk the scaffold from the base up and mark Pass, Fail, or N/A on each line.
  3. If any line is Fail, close that part of the scaffold, put a "Do Not Use" tag on it, and write the defect and fix in the defect record.
  4. Before the scaffold is used again, the competent person checks the fix and signs the re-check.
  5. Keep each sheet with the job file. Start a new sheet for every inspection.

Adapting it to your site

  • Delete lines that do not apply, such as suspended scaffold parts or debris netting.
  • Add lines from the scaffold manufacturer's manual, such as bracket loads, hoist arms, or a specific tie pattern.
  • If you use a green / yellow / red tag system, write the tag color in the summary.

Before you use it

Please read
  • OSHA 29 CFR 1926.451(f)(3) asks for scaffolds and components to be inspected for visible defects by a competent person before each work shift and after any occurrence that could affect structural integrity. This template is a way to record that; it is not an official form.
  • Heights, spacing, and load limits differ by scaffold type. Follow the manufacturer's manual and erection drawing, and the rules that apply to your scaffold.
  • Do not use a scaffold with a Fail line until it is fixed and the competent person has checked it again.
  • State plans may add requirements. Check the current rules that apply to your site.
  • This template is provided as-is for reference. Worker Library is not liable for any loss arising from its use. Regulations change, so check the update date on this page.

Scaffold Inspection Checklist FAQ

How often does a scaffold need to be inspected?

OSHA 29 CFR 1926.451(f)(3) says scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect structural integrity.

Who can inspect a scaffold?

Under 29 CFR 1926.451(f)(3), the inspection is done by a competent person. Write that person's name in the header of each sheet.

Do I need to keep scaffold inspection records?

The rule above asks for the inspection; it does not set a form. Many contractors keep a written record for each inspection with the job file until the job ends. Check your company policy and the general contractor's requirements.

Does this checklist work for rolling scaffolds?

Yes. The Rolling Scaffold sheet has 37 items for scaffold towers on casters, including caster brakes, outriggers, and what to check each time the tower is moved.
