Who is this quality inspection report for?
- Machine operators and inspectors who measure parts during a production run
- Quality engineers who need first-piece, in-process, and last-piece checks in one record
- Shops that need to trace an out-of-spec part back to the gauge and the time it was found
When to use it
Use one sheet per part number per day or per shift. Write a row at each scheduled check, such as the first piece after setup, every 2 hours during the run, the first piece after a tool change, and the last piece.
What's included
The template includes the following. Delete anything you do not need.

| 7 | ■ Sheets in this workbook |
| 8 | "Entry Sheet" … The sheet you fill in on a computer. Landscape layout. |
| 9 | "Sample (Filled In)" … A filled-in example. Use it as a guide to how each column is written. |
| 10 | "Print (Letter)" … For printing and filling in by hand. Taller rows, and the column headings repeat at the top of every page. Set up for US Letter. |
| 11 | |
| 12 | ■ How to use |
| 13 | 1. Fill in the process, part, and date at the top. |
| 14 | 2. At each check, write the time, lot, inspection type, and the characteristic with its LSL and USL from the drawing. |
| 15 | 3. Measure, write the value, and mark Pass or Fail. Write the gauge ID and check that its calibration date has not passed. |
| 16 | 4. For a Fail, stop and write what you did: the cause, the parts you re-inspected since the last good check, and the disposition. |
| 17 | 5. At the end of the shift, the supervisor reviews and signs the sheet. |
| 18 | |
| 19 | ■ How to mark results, and what to do (read before you start) |
| 20 | Pass … OK to use. |
| 21 | Fail … Stop using it on the spot and tag it "Do Not Use" or barricade the area. Tell your supervisor and write the corrective action. |
| 22 | N/A … The item does not exist here or does not apply today. Do not leave it blank. A blank cell means "not checked". |
| 23 | The inspector may decide to stop. Nobody is blamed for stopping. If in doubt, mark Fail. |
| 24 | After the corrective action, do not return it to service until the verifier writes the re-check date and name. |
| 25 | |
| 26 | ■ Adapting it to your site |
| 27 | • Change the number format of the limit and measured columns to match your drawing, for example 3 decimals for 0.001 in. |
| 28 | • Add a nominal column, or a column for the machine or operator, if you need them. |
| 29 | • For many characteristics on one part, list them in the same order as the balloon numbers on the drawing. |
| 30 | |
| 31 | ■ Things to watch |
| 32 | • This is a general quality inspection template. It is not a form from any customer, standards body, or agency. |
| 33 | • What to measure, how often, and how long to keep records depend on your control plan, your customer's quality requirements, and your quality system (for example ISO 9001 if you are certified). |
| 34 | • Do not use a gauge past its calibration due date. Tag it and send it for calibration. |
| 35 | • When a part fails, re-inspect every part made since the last good check, not only the one you measured. |

Quality Inspection Report FAQ
What is a quality inspection report?
It is a record of the checks made on a product: what was measured, the spec limits, the result, who checked it, and what was done when a part failed. This template is set up for in-process checks during production.
What should be included in a quality inspection report format in Excel?
At least the part and lot, the characteristic, the spec limits, the measured value, Pass or Fail, the gauge used, the inspector, and the action for any failure. This template has a column for each, plus the gauge calibration due date.
What is the difference between first piece, in-process, and last piece inspection?
A first piece inspection checks the first part after a setup or tool change. In-process inspections are done at set intervals during the run. A last piece inspection checks the final part before the setup is torn down.
Can I see a sample quality inspection report?
Yes. The Sample sheet shows a filled-in day with a bore and a length check, one out-of-spec result after tool wear, and the action taken.
