How to use the nonconformance report log

  1. Fill in the site, who keeps the log, and the period at the top.
  2. When a nonconformance is found, tag and hold the parts, then add a row with the next NCR number, the date, where it was found, and what was found.
  3. Enter the qty nonconforming and qty inspected. The defect rate fills in by itself.
  4. Write the containment and the disposition. Then find the root cause and write the corrective action with an owner and a due date.
  5. After the action, check that the problem did not come back and write the date and result. Only then is the row closed.

Adapting it to your site

  • Add a column for a CAPA number if corrective actions are tracked in a separate system.
  • Add a column for cost (scrap, rework hours) if you report the cost of poor quality.
  • Add a "Source" drop-down (incoming, in-process, final, customer) to sort the log by where problems are found.

Before you use it

Please read
  • This is a general nonconformance log template. It is not a form from any customer, standards body, or agency.
  • How you handle and record nonconforming product depends on your quality system (for example ISO 9001 if you are certified) and your customer's quality requirements.
  • A "Use as is" disposition accepts parts outside the spec. Get approval from the person allowed to sign it, and from the customer if they require it.
  • Do not close a row when the action is done. Close it when the effectiveness check shows the problem did not come back.
  • This template is provided as-is for reference. Worker Library is not liable for any loss arising from its use. Regulations change, so check the update date on this page.

Nonconformance Report Log FAQ

What is a nonconformance report (NCR)?

An NCR is a record that a part, material, or product does not meet its requirements. It describes what was found, how many parts are affected, what was done with them, and the corrective action to stop it from happening again.

What should a non conformance report template include?

Most NCR forms include an NCR number, date, part and lot, a description of the problem, quantity, root cause, containment, disposition, corrective action, owner, and a check that the action worked. This log has a column for each.

What are the usual NCR dispositions?

Common dispositions are use as is, rework, repair, scrap, and return to vendor. Use as is and repair usually need approval because the part does not fully meet the spec.

What is the difference between an NCR and a CAPA?

An NCR records one nonconformance and what was done with the affected parts. A CAPA (corrective and preventive action) is the process of finding the root cause and making sure it does not happen again. This log keeps both on the same row.
